Demodicosis is the most likely option, if you consider history, breed and clinical signs of the patient. And in all skin scrapings and the cerumen sample you find multiple living Demodex canis mites of all stages.
How do you proceed?
a) You treat the demodicosis with ivermectin injections at weekly intervals
b) You give weekly ivermectin injections combined with topical benzoylperoxide shampoo
c) You tell the owner that the patient has a non curable disease and recommend euthanasia
d) You tell the owner, that the dog has a generalised demodicosis and recommend to look for the underlying course of immunsuppression (spontaneous or iatrogenic)
